Hospice is a compassionate way of providing health care to individuals coping with a life-threatening illness. At North Memorial Hospice, we care for both patients and their families by providing services wherever an individual resides:

  • At home
  • In a skilled nursing or assisted living facility
  • On the Hospice and Palliative Care Inpatient Unit at North Memorial Medical Center
  • At the North Memorial Residential Hospice (or other hospice residences).

Relief of a patient's physical symptoms is our first priority. That is why the most advanced symptom control and pain management methods are used. Yet, hospice care goes far beyond simply meeting one' s physical needs. It also involves emotional and spiritual support for patients and families. Patients are encouraged to live each day to the fullest and to embrace the love of family and friends. Our staff meets the needs of patients and their loved ones with skill and sensitivity.

Home Care
Home is the place that is so essential and important all of our lives. Even when faced with serious illness, hospice patients can be in the comfort of their own home. The North Memorial Hospice Home Care staff provides support, knowledge, teaching, and care to those who desire living at home and their families.

Skilled Nursing and Assisted Living Facilities
Our Hospice Home Care staff provides visits and brings symptom management expertise, to patients in the skilled nursing and assisted living facilities. North Memorial Hospice staff work hand-in-hand with the facility staff, sharing the goal of providing physical, emotional and spiritual support for the residents and their families.

Inpatient Hospice and Palliative Care Unit
Working with your physician, the Inpatient Unit staff addresses issues such as the adjustment to changing physical conditions, medication needs, symptom control and care requirements. Patients and their families will experience a calm and reassuring atmosphere, a welcoming environment and experienced staff with knowledge of what can increase comfort. Care is tailored to the individual needs of patients and families.

This eight-bed Residential Hospice is located on two acres of lake property on Upper Twin Lake, in Brooklyn Center. It is a welcoming and inviting home in the community for patients who need 24-hour care, are no longer able to care for themselves, have no family to provide the care, or prefer a more intimate, home-like atmosphere outside of their own residence. The eight private rooms all have a view of the lake, a private bathroom and are equipped with a sofa sleeper for family members' comfort. Care is provided by skilled hospice specialists. These specialists address concerns and provide 24 hour nursing care, allowing family members to focus on the role of being a family member.

Our Hospice Team

The interdisciplinary hospice team works with the patient and their family to create a personalized plan of care for both patients and families. The plan of care will address physical, emotional and spiritual needs with a focus on comfort.

Depending upon the unique and individual needs of the patient, hospice care team members may include:

Hospice Volunteers

Trained volunteers are highly valuable team members providing services to our patients and their families. These supervised volunteers care for our patients and their families in a variety of ways:
Respite for caregivers. Offering time to sit with the patient while family takes time to run errands, attend necessary appointments or get some needed rest.
Socialization for the patient. Offering time to play board games, cards, read, write letters, go for walks, converse or just sit quietly with patients. Accompanying patients to various activities or events within the skilled nursing or assisted living setting.
Emotional support. Offering conversation and practicing good listening skills with patients in all of the hospice care settings.
Hospitality. Greeting and welcoming family members and visitors at the North Memorial Residential Hospice.
Administrative. Assisting with mailings and special projects.

Commonly Asked Questions About Hospice

When should a decision to begin hospice care be made and who should make it?
At any time during a life-limiting illness, it is appropriate to discuss all of the patient's care options including hospice, so that informed decisions can be made. Patients and family members can discuss hospice care at any time with their physician, other health care professionals, clergy or friends. Is hospice care only for cancer patients?
No. Anyone whose illness is known to be life limiting and whose life expectancy is less than six months is eligible for hospice care. Many patients we serve have a non-cancer diagnosis such as end stage heart disease, lung disease, renal failure or liver disease.

How is hospice care paid for?
Hospice services are paid for by Medicare, Medical Assistance, HMOs and most private health insurance plans. North Memorial Hospice staff will work with each patient and family to identify payer sources.

Do I have to stop seeing my own physician?
No. Patients are encouraged to continue receiving care from their attending physician. North Memorial Hospice will work with your personal physician to coordinate an individualized care plan.

Guidelines for North Memorial Hospice Admissions

The patient must meet the following criteria:
Patient has diagnosis of a life limiting illness, usually six (6) months or less as certified by their physician.
Patient desires hospice care.
Patient lives in the North Memorial Hospice service area of Minneapolis and surrounding suburbs. Call the Intake and Referral Nurses at 763.520.3917 to clarify.
Patient will have a primary contact person who will be responsible for decision making if the patient is no longer able.
