Chronic Condition Self-Management Program | Tuesdays, March 18-April 22, 2025 | 2-4:30pm
This is a six-week class developed at Stanford University’s Patient Education Research Center and offered around the world. The class has helped participants with ongoing health conditions such as arthritis, diabetes, high blood pressure, anxiety, osteoporosis, heart disease and other chronic conditions to:
The class is taught by two trained peer leaders and will meet for 6 weekly sessions of 2.5 hours each week. Each class includes a new topic designed to help participants dealing with chronic conditions. The class is a combination of presentations, discussion, and activities with others who have similar experiences. Participants make weekly action plans, share experiences, and help each other solve problems they encounter in creating and carrying out their plan.
Must have a minimum of 6 participants in order to hold the class. Register here.
