Minnesota Healthcare Program Renewal
If you are on Medical Assistance or MinnesotaCare through the state of Minnesota, your health insurance needs to be renewed periodically. As your healthcare provider, we want to make sure you know you will have to take some steps to avoid losing your health insurance and creating possible disruptions in seeing your doctor or filling a prescription. You’re going to receive a packet in the mail with information about how to keep your health insurance and the steps needed to renew your coverage.
Frequently asked questions for enrollees
A renewal is a review of your income and other information to see if you and other family members are still eligible for Medical Assistance (MA) or MinnesotaCare.
Most MA and MinnesotaCare enrollees have a renewal once a year.
To keep your health insurance during the COVID-19 pandemic, renewals were stopped. Renewals are going to start again.
A renewal is needed to review your eligibility to see if you still qualify for health insurance coverage.
If you have MA, you can expect to get renewal forms from DHS about two months before the month you originally applied for health insurance. For example, if you applied in July, your MA renewal forms will be mailed to you in May. Do not worry if you don’t remember when you applied. DHS will mail renewal forms to you when it is time to renew your MA. If you have MinnesotaCare, you can expect to get your renewal forms in October 2023.
About six weeks before mailing your renewal forms, DHS will mail you a letter telling you how to get ready and to watch for your renewal in the mail. When it is time for your renewal, DHS will mail your renewal forms and instructions.
You will get your renewal forms in the mail. DHS will send it to the mailing address they have on file for your MA or MinnesotaCare case.
If you moved, make sure to report your new address right away. If you have MA, or your household has both MA and MinnesotaCare, call your county or tribal agency. If you have MinnesotaCare, call DHS Health Care Consumer Support at 651-297-3862 or 800-657-3672.
You can get help with completing your renewal form by:
- Calling or visiting a MNsure Certified Navigator. They can help answer your questions or help you fill out the renewal form. Go to the Assister Directory to find a navigator that can help you and that speaks your language.
- If you have MA, contact your county or tribal agency for help.
- If you have MinnesotaCare, or your household has both MA and MinnesotaCare, contact DHS Health Care Consumer Support at 651-297-3862 or 800-657-3672.
Yes. Send proof of your income with your renewal form. It may help DHS process your renewal more quickly. Include:
- Pay stubs for people who have wages
- Most recent federal income tax return or business records for people who are self-employed
- Documentation or copies of checks if you have other kinds of income
Return it to the agency listed on the cover letter that came with your renewal form. If you no longer have the cover letter and you have MA, contact your county or tribal agency. If you no longer have the cover letter and you have MinnesotaCare, or your household has both MA and MinnesotaCare, contact DHS Health Care Consumer Support at 651-297-3862 or 800-657-3672.

Yes, if you want DHS to decide if you still qualify or see if you are eligible for a different health insurance program.
Yes. Complete and return your renewal form as soon as you can, even if your coverage ended. DHS will look at your case again if we get your renewal form within four months after your coverage closed. After that, you will need to reapply.

- Renewal forms are processed in the order they are received at your county or tribal agency or at DHS.
- Your renewal will take longer if it is not complete or not signed. Answer all the questions and sign your renewal form to help us process it faster.
- DHS will notify you when your renewal form has been processed. DHS will send a notice if we need more information from you to decide your eligibility. DHS will send a notice to tell you whether your eligibility has been renewed.
- Continue to pay the amount shown on your invoice. Your MinnesotaCare premium may change after your renewal form is processed.
- The premium notices you get in November and December may show a premium amount of “unknown” instead of a dollar amount. If you already returned your renewal form, this means that it hasn’t been processed or received or the agency is waiting for more information from you. If you have not returned your renewal form, you must return it by the due date or your eligibility will end December 31.
