- North Memorial Health has trained team members to help patients with Advance Care Planning. They can also help you write a legal document called a Health Care Directive.
- You may schedule an appointment with one of our advance care planning facilitators available at our two hospitals, our primary care clinics, the North Memorial Health Cancer Center, or any of our North Memorial Health Heart & Vascular locations.
- Advance care planning and finishing health care directives are associated with easing the burden on family members and people you trust to make decisions for your care if you are unable.
