Submitting your form via MyChart ensures that the information you provide is completely secure. One of our nurse navigators will evaluate your information so that your care team can plan for your delivery. This form should be completed at least six weeks prior to your due date.

If you have a MyChart account with another healthcare system or hospital, your username and password will not work and you will need to create a new account with the North Memorial Health MyChart.

Steps for registering your delivery:
- Enter your North Memorial Health MyChart username and password on the homepage of North Memorial Health MyChart. Forgot username? Forgot password?
- Complete pre-registration in MyChart by clicking “Your Menu” and selecting “Register for My Delivery.” Information to be prepared for filling out the form:
- You will need all of your insurance policies, baby’s intended insurance, information for the person responsible for the bill. Please have that information readily available.
- Please allow for approximately 15-20 minutes to fill out the form.
- Submit your completed form.

- Select the "Sign up now" button on the homepage of North Memorial Health MyChart. You will be asked several questions so that we can verify your identity in effort to ensure that your information will be completely secure. Once we have validated who you are, you will be able to create a username and password to access MyChart and the registration form. If you have issues setting up a new account, call 763-220-1192.
- Complete pre-registration in MyChart by clicking “Your Menu” and selecting “Register for My Delivery.” Information to be prepared for filling out the form:
