Home Care and Physical Therapy
Many patients return home immediately after their stay at the Joint Replacement Center. They may be referred to North Memorial Home Health Care or a transitional care unit. Each patient's specific needs are considered, including nursing, home health aides, and physical therapy.
Upon leaving the Joint Replacement Center, patients are given detailed information and instructions for home exercises. To help make a smooth transition back to a normal level of activity, some may be referred to physical therapy.
