Appointment Options

eVisit Appointment through MyChart

  • MyChart eVisits provide an easy avenue to get a diagnosis, treatment plan, and prescription for common health conditions within 24 hours.
  • Available seven days a week from 8 a.m. – 7 p.m. through MyChart
  • A $49 fee will be pre-authorized on your credit card but only applied if a health condition is diagnosed and treated online. Payment is required upon diagnosis.
  • North Memorial Health does not submit eVisit bills to your insurance. Insurance reimbursement varies by plan.

Primary Care

North Memorial Health primary care clinics focus on health maintenance, wellness, and managing chronic conditions, building a lifetime of trust and comprehensive health care between you and your providers, right in your own neighborhood

Primary care is the place to turn when you need:

  • Preventive care for the whole family, including children: disease prevention, family planning, health education and promotion, wellness visits, and more.
  • Diagnosis and treatment of acute illness such as cold & flu, ear infections, and the like.
  • Management of commonly seen chronic conditions, such as asthma, anxiety, COPD, depression, diabetes, hypertension, thyroid dysfunction, and more.
  • Physical exams, including sports physicals.
  • Vaccines.
  • Lab work: blood glucose, blood tests, pap smears, pregnancy, STI testing, urinalysis, and more.

Preparing for Your Visit

Prior to your appointment, here are some helpful tips:

  • Please arrive 15 minutes early for your visit
  • Bring all of your medication bottles or a current list of medications
  • Pay out-of-pocket expenses before you arrive or upon arrival to your appointment using MyChart. If you have questions about payment options, talk to your care team at your appointment or call our business office at your convenience at 763-581-4479.
  • Please note, we require a 24 hour cancellation notice
  • Complete the questionnaire below prior to your visit to allow for a quicker check in
