Preparing for Your Visit
- If you were provided any paperwork prior to your visit, please bring the completed documents with you.
- Bring a current list of all your medications, or the actual bottles, including any needed heart medication refills.
- Please check to see if you were given any instructions on what type of clothing to wear to your appointment.
- Please remember to bring any applicable insurance cards, photo ID and copayment.

Virtual Doctor Visits
- Telephone visits are available for select specialty care conditions including mental health therapy. Visits are available five days a week from 8 a.m. to 5 p.m. Contact your specialty clinic to identify if the care you need can be delivered over the phone.
- Video visits are available five days a week from 8 a.m. to 5 p.m. with select specialty care teams including Concierge Medicine, Infectious Disease, Endocrinology, Heart and Vascular, Mental Health, Pain Management, and Rheumatology.To participate in a virtual visit, you will need the following:

Pharmacy & Lab
North Memorial Health Pharmacy is integrated with our electronic health records, ensuring your needs are up-to-date. Staffed by board-certified pharmacists, you can feel confident our pharmacists understand your pharmaceutical needs and how your medications and treatments interact, help you manage side effects, and keep a specially trained eye on your regimen. We work with your care team to order refills, provide refill access by phone, mobile device or online, and work with your provider on medication coverage authorizations, when necessary. The pharmacy is located on the first floor for convenient pick-up after your visit.
We also offer the ultimate convenience with our on-site lab. High-quality, full service, cost effective laboratory services with state-of-the-art instrumentation offered with remarkable customer service. Interpreter Services
We offer support options for deaf, hard of hearing, language assistance and translation services. These services are all free of charge and available 24 hours a day, seven days a week for patients at North Memorial Health.
